Transportation Disclosure

I understand that Fresh Start Worship Center and their members are not liable in case of an accident. I understand that all precautions will be taken in order to provide safe and worthwhile transportation. I also understand that if my children and or adults that do not follow van/bus guidelines may be informed that they are no longer allowed to ride. In the event I cannot be reached to make arrangements for emergency medical care at time of an accident, I hereby authorize Fresh Start Worship Center to take my child to the nearest hospital with emergency care.
